Responsibilities:
Responsibilities:
- Evaluate, plan, and implement individualized treatment plans for residents.
- Collaborate with an interdisciplinary team to achieve patient goals.
- Provide therapeutic interventions to improve mobility, strength, and overall function.
- Document progress and maintain accurate records in compliance with regulations.
- Educate residents and families on continued care post-discharge.

Qualifications:
- Active Physical Therapist (PT) license in Delaware.

- Experience in a skilled nursing or rehabilitation setting is a plus, but new graduates are encouraged to apply.
