Job Description

Join the Medadventures team as a Physical Therapist in a dynamic Skilled Nursing Facility (SNF) setting. We’re seeking a dedicated PT to help improve the mobility and quality of life for a diverse group of long-term care and rehab patients.

About the Role:
In this position, you’ll work closely with a multi-disciplinary team to assess and treat patients with varying levels of physical function. Your goal will be to enhance their strength, balance, and mobility, enabling them to regain independence and improve their quality of life.

Key Responsibilities:

  • Conduct comprehensive patient evaluations
  • Create and execute personalized treatment plans
  • Deliver one-on-one therapy sessions focusing on functional outcomes
  • Collaborate with other healthcare professionals to deliver patient-centered care
  • Document patient progress and adjust treatment plans as necessary
  • Educate patients, families, and caregivers on therapy exercises and safety protocols

Qualifications:

  • Licensed Physical Therapist (PT) in [State]
